How to Apply
Churchill is designed specifically for, and accepts only, high potential children
whose primary diagnosis is a learning disability. Students are admitted on the
basis of their potential for success in the program. Each candidate is evaluated
on an individual basis. The final responsibility for all admissions decisions
rests with the Director. Churchill offers nonsectarian education to any qualified
student regardless of national origin, race or religion.

Application Requirements
- A psycho-educational evaluation, administered within the last three years, is required as a part of the application. Evaluations from an educational agency, medical facility or private practitioner are acceptable. The Wechsler Intelligence Scale for Children (WISC III or IV) is a necessary part of the evaluation. Subtest and index scores from the WISC must be included. If the results of any information or testing prove inconclusive or ambiguous, the Admissions Committee may request additional information.
- A completed application packet should be submitted to Churchill along with the application fee of $50.00. The fee is refundable only if the program is found to be inappropriate for the applicant.
- The Release of Information forms should be sent to the schools, doctors or clinics that are familiar with your child. DO NOT SEND THE RELEASE OF INFORMATION FORMS TO CHURCHILL. The School Information form should be signed by the parents and sent directly to your child's school. The school will be responsible for sending the completed form to Churchill.
- It is the responsibility of parents to ensure that all pertinent information is forwarded to Churchill. Please list on the Student Information form the persons to whom you are sending release forms.
- A personal interview with the parents and the applicant is required prior to admission. This can only be scheduled after the school has received all application materials.

- Application can be made at any time, although early application is advised. Students are accepted on a space–available basis.
- As soon as your child's file is complete, an interview will be scheduled if the Admissions Committee deems that Churchill is an appropriate placement for your child. Interviews continue until enrollment is filled for the following summer or fall.
